Apply On-Line for NADP Membership

The NADP Membership Criteria describe NADP categories of membership.  You may apply on line for any membership category.

If you are a Dental Plan (Full Voting) member applicant, to apply on-line you will need to have enrollment (subscribers plus dependents) for the following lines of business available to complete your application:  dental HMO (fully insured & self-insured), dental PPO, dental Indemnity, Discount Dental, ASO (all products except dental HMO), and Medicaid.  Other categories of membership require basic information only.

If you would like to apply on-line use the link below to access our on-line membership process.  Once submitted your application will be reviewed by NADP staff and when accepted, an invoice for membership will be sent to you via e-mail.
